Watch for your Medicaid redetermination forms in the mail from the State of Illinois. You can complete your forms online by scanning the QR code or visiting abe.illinois.gov, or attend one of several redetermination events being held at various locations and times for in-person assistance. Humboldt Park Health is proud to serve as one of the event sites to support community members through this process. The Humboldt Park Health event will take place on February 25th from 10:00 a.m. to 2:00 p.m. at the Innovation Center. Please bring your redetermination paperwork and proof of income from the last 30 days (if you are employed).
