HPH is dedicated to providing the Humboldt Park community with quality behavioral health services that are comprehensive and evidence based. Through an interdisciplinary approach, we offer treatment and counseling for patients with mental health and substance abuse issues.
Behavioral Health Overview
Substance Abuse Treatment
Humboldt Park Health Outpatient Behavioral Health Clinic provides alcoholism and drug abuse treatment programs for adults who are demonstrating problems or potential problems with either drugs or alcohol. Humboldt Park Health provides drug-free counseling and treatment alternatives with MAT (medication assisted addiction) using Suboxone to treat opioid addiction and providing methadone referrals for those who prefer methadone. Through comprehensive and continual assessments, the patient is engaged in the treatment process. Services include assessment, treatment plan and individual and we will be providing group counseling. We provide services for patients who want to learn how to recover from alcohol or other drug use disorders. We will assess and refer to an appropriate level of care from detoxification to outpatient counseling or residential treatment and we provide case management services to help patients learn how to re-engage back into mainstream society. We deliver substance use disorder treatment assessments and counseling that require weekly to bi-weekly attendance at a clinic or facility, allowing the patient to return home or to other living arrangements during non-treatment hours.
Medication Education & Medication Management
Humboldt Park Health Outpatient Behavioral Health Clinic wants to educate for our patients by providing information, education and giving the patients the option to decide about how they want to approach their health and also to be part of the process of getting healthier day by day. Our team will talk with the patients about medicines in a way that is meaningful to them as a person with mental health, we will educate the person about their options about side effects experienced from the medicine and we will ask how they are doing on medication treatment through weekly or bi-weekly counseling. We want to help patients understand the importance of taking the exact dose prescribed to help eliminate hospitalizations. Mental health disorders require a two pronged approach: medication combined with therapy.
Case Management
Humboldt Park Health Outpatient Behavioral Health Clinic provides substance abuse and mental health case management services to help patients connect to resources to improve the quality of their lives. We prioritize the bio-psychosocial needs of the patient taking into consideration there social and economic circumstances. We work with the patient to develop treatment plans to set goals and objectives based on the patient’s needs. We provide an assessment of the patient’s triggers, strengths, and personal needs. We coordinate with several resources and other public or free-of-charge care providers to help patients realize their personal goals.
Counseling Services
Humboldt Park Health Outpatient Behavioral Health Clinic provides counseling to help promote patients mental health and wellbeing. Patients are counseled for there mental and substance use disorders to provide support of those who experience and are in recovery from these conditions. Furthermore it provides support for patient’s families and communities. Therapy can help identify and help change potentially self-destructive or unhealthy behaviors for the patient and our communities.
Cancer screening tests help check for cancer even when you aren’t showing symptoms. When cancer is caught early, it may allow for more treatment options and better outcomes.
Cancer Screenings Available at Humboldt Park Health
- Breast cancer screening (annual mammograms): Recommended yearly beginning at age 40
- Cervical cancer screening and HPV testing (Pap smears): Recommended every 3 years beginning at age 21
- Colorectal cancer screening (colonoscopies): Recommended beginning at age 45
- Lung cancer screening (low-dose CT scans): Recommended for those age 50 or older who are current smokers or quit in the past 15 years, and have a 20-pack year smoking history
- Prostate cancer screening (PSA testing): Recommended beginning at age 50
- Skin cancer screening (skin checks): Recommended yearly beginning at age 21
Screening recommendations are for those considered at average risk. However, it is important to talk to your primary care doctor to understand your individual risk factors and if a different screening schedule is appropriate for you.

Cardiac Catheterization is a diagnostic and interventional procedure that involves inserting a thin, flexible tube (catheter) into the heart through a blood vessel. It is used to assess the structure and function of the heart, diagnose heart conditions, and perform certain treatments.
Cardiology is dedicated to the study, diagnosis, treatment, and prevention of diseases and disorders related to the heart and the entire cardiovascular system (heart and blood vessels). Cardiologists perform a variety of tasks, which can range from conducting physical exams and reviewing medical histories to performing specialized diagnostic tests and procedures.
Dermatology focuses on the diagnosis and treatment of conditions that affect the skin, hair, and nails. Our specialists provide comprehensive care for everything from routine check-ups to specialized treatments
Diabetes Learning Center
The Humboldt Park Health Comprehensive Diabetes Center provides outpatient information, support and teaching for adults and children diagnosed with diabetes. This includes persons diagnosed with Type I, Type II, Pre-diabetes, and gestational diabetes. The Center delivers diabetes education by a certified diabetes educator and registered dietitian. The Center assures high-quality education to help develop and improve patient self-care skills essential to diabetes management and is recognized by the American Diabetes Association. Family members are encouraged to participate. Our staff provides education to those newly diagnosed with diabetes or those needing assistance in gaining better control of their diabetes.

The Humboldt Park Health Gastrointestinal (GI) Lab performs diagnostic and therapeutic procedures related to the digestive system. The primary purpose is to identify, treat, and manage a wide range of conditions affecting organs from the esophagus to the rectum, including the stomach, intestines, liver, gallbladder, and pancreas.
General surgery focuses on the diagnosis, preoperative, operative, and postoperative management of a broad spectrum of diseases and conditions requiring surgical treatment across various areas of the body.
Hematology is focused on the study, diagnosis, and treatment of diseases related to blood, blood-forming tissues (bone marrow), and the lymphatic system.

Computed Tomography (CT) Scan is a diagnostic medical imaging procedure that combines a series of X-ray images taken from multiple angles around the body and uses computer technology to create detailed, cross-sectional images (slices) of internal structures.
Mammography is a specialized medical imaging technique that uses low-dose X-rays to create detailed pictures of the breast tissue, known as a mammogram. It is the most common and effective screening tool for the early detection and diagnosis of breast cancer and other breast conditions, often years before physical symptoms are apparent.
An MRI (Magnetic Resonance Imaging) is a medical imaging technique used in radiology to form pictures of the anatomy and the physiological processes of the body in both health and disease. MRI scanners use magnetic fields, magnetic field gradients, and radio waves to generate images of the organs in the body.
Nuclear Medicine is a specialized medical field that uses small, safe amounts of radioactive materials, called radiotracers or radiopharmaceuticals, to assess organ function, diagnose diseases, and treat conditions like cancer and thyroid disorders.
Ultrasound is a non-invasive medical imaging technique that uses high-frequency sound waves to create real-time pictures (sonograms) of the organs, tissues, and structures inside the body. Unlike X-rays or CT scans, it does not use ionizing radiation, making it a safe method, especially during pregnancy.
An x-ray is a form of high-energy electromagnetic radiation used to create images of the inside of the body or other opaque objects. The imaging process, also called radiography, works because different materials absorb varying amounts of radiation, resulting in images with different shades of black, white, and gray.
The Infectious Disease department specializes in the diagnosis, treatment, control, and prevention of disorders caused by microorganisms such as bacteria, viruses, fungi, and parasites. Maternal & Child Health focuses on promoting and improving the physical, mental, and social well-being of women during pregnancy, childbirth, and the postnatal period, as well as the health of infants, children, and adolescents. Same Day Surgery, also known as outpatient surgery or ambulatory surgery, refers to a surgical procedure that allows the patient to return home on the same day they have the operation, without an overnight hospital stay.
Benefits of Same Day Surgery:
- No Overnight Stay: The primary defining feature is that the patient is discharged from the hospital or surgical facility within a few hours of the procedure.
- Minimally Invasive: These procedures are often less invasive, involving smaller incisions and less post-operative pain, which allows for a quicker initial recovery.
- Convenience and Cost-Effectiveness: Recovery at home is often more comfortable and less stressful than staying in a hospital. Not requiring an overnight stay also significantly reduces costs.
The Stroke Program is designed to provide rapid, standardized, and high-quality care for stroke patients, from emergency response through diagnosis, treatment, and rehabilitation. The primary goal is to improve patient outcomes, long-term disability, and prevent future strokes.
Urology is the medical and surgical specialty focused on the diagnosis, treatment, and management of conditions affecting the male and female urinary tract and the male reproductive system. The urinary tract includes the kidneys, ureters, bladder, and urethra, as well as the adrenal glands located on top of the kidneys.
The Women’s Health clinic provides comprehensive medical services for women at all life stages, from reproductive and gynecological care to obstetrics, preventative screenings, and management of chronic conditions. These services include annual exams, cancer screenings, contraception, prenatal and postpartum care, and management of conditions like menopause.
The Wound Clinic is a specialized medical facility dedicated to the diagnosis, comprehensive care, and treatment of chronic or complex wounds that have not healed through typical methods within a normal timeframe. The primary goal is to promote healing, prevent infection and complications, and improve the patient’s overall quality of life.
