HPH Family Medicine Residency

Curriculum

Beyond the core requirements for Family Medicine set forth by the ACGME, the curriculum is organized in a unique layering of experiences that provides the learners with timely exposure and reinforcement of medical subspecialties as their continuity clinic responsibilities evolve. Additionally, residents are given scheduled time to undertake quality improvement projects, engage in community outreach, and develop practice management skills in a longitudinal fashion through the TEAM Care rotations. Finally, a midyear session known as “Interlude” creates opportunities to learn new procedure skills, explore the humanities, and take time for self-care and wellness. ​ The grid below details the rotation requirements for Family Medicine Residents: (13-4 week blocks)

One to two half-days of continuity of care clinic in the PGY-1 year
Two to three half-days of continuity of care clinic in the PGY-2 year
​Three to four half-days of continuity of care clinic in the PGY-3 year

Application Requirements

Must be a graduate of an accredited medical school. USMLE Scores: passing scores on Step 1, Step 2 CK, and CS Foreign applicants must be ECFMG certified. Hey everyone, my name is Hamza, and I’m from Pakistan. I completed my medical school in Pakistan before moving to Chicago. I enjoy playing soccer, chess and video games. In my free time, I like working out, going on long drives and trying out new restaurants. After living my life in hot climate countries, i am excited for this next phase of living in the windy city and acquiring further medical training here!

Hey everyone! My name is Semal Musleh and I was raised in Minnesota. I went to medical school at St. George’s  University and I completed the second half of medical school here in Chicago, and I’m excited to continue working here! I love trying new restaurants and enjoying nice weather in Chicago with friends and family.

Adelis Arias, MD

Hola todos! I’m Adelis; raised in Valencia, Venezuela, and attended Universidad de Carabobo. Upon graduation, I practiced as a rural physician in several underserved areas. I moved to Chicago in 2016 to pursue residency training in the US. I’m passionate about working with underserved populations, minorities, and lifestyle medicine. In my free time, I enjoy working out, grilling, and watching the latest movie/tv show. I’m thrilled to spend three more years in this beautiful city that I now call home. I am looking forward to meeting you all soon, go Cubs! 

Mackenzie Bradley, MD:

 Hi I’m Mackenzie! I was born and raised in Buffalo, NY (Go Bills!). I studied Chemistry at Binghamton University before attending medical school at St. George’s University. My husband and I moved to Chicago in 2021 while I completed my clinical rotations at Humboldt Park Health Hospital. I fell in love with the community, patients, and especially my coworkers which made it an easy choice to stay for Residency! My interests include community medicine and adolescent medicine. In my free time, I enjoy hiking, knitting/crocheting, hobby photography, and playing with my one-eyed dog, Jack. A fun fact about me is that I speak German!

Zil Gharia, MD

Zil grew up in the suburbs of Chicago and went to Loyola University Chicago for Undergrad, majoring in Biology and minoring in Psychology. She obtained her medical degree from St. George’s University in Grenada. Being from the Chicagoland area, Zil was excited to return back home and be near family and friends during residency. Zil is still contemplating what specific interest of medicine she wants to join. In her free time, Zil likes to workout, spend time with her friends, and go on spontaneous adventures. Fun fact about Zil is that she is trying to become a Mocha snob and find the best mocha in town. 

Kush Patel, DO

Hi everyone! My name is Kush Patel, aka “KP”. I was born in Ahmedabad, India and I grew up in Toronto and yes I do love my fair share of Tim Horton’s, my favorite pastry is their Pecan Danish. I moved to Chicago during my Sophomore year of high school and I completed both my Bachelor’s and MPH at UIC before attending the Chicago College of Osteopathic Medicine at Midwestern University for medical school. In my free time, I like to find new songs to add to my Spotify playlist as well as trying new restaurants in Chicago, especially new pizza places. I enjoy playing pickup basketball and I love watching basketball and football. I am a huge Cubs and Bulls fan, however my favorite basketball player of all time is Lebron James! I am excited to join the team at Humboldt Park Health and help advance health literacy and equity in the underserved community of Humboldt Park.

Shainy Raj, MD

Hi, I’m Shainy! I am originally from India and moved to Canada when I was 12. I pursued my undergraduate studies at the University of Toronto and earned my medical degree from St. George’s University School of Medicine. My clinical rotations led me to Chicago, where I had a great experience working with a fantastic healthcare team at HPH, providing care for underserved communities. I’m thrilled to join the same team as a family medicine resident. Outside of medicine, I enjoy painting, hiking, and exploring different cafes around the city. I look forward to spending the next 3 years in this great city. 

Hamid Hashmat, MD

Hello! I’m a first-generation Afghan-American, originally from Tampa, FL, and now a new resident of Chicago. I’m looking forward to experiencing the stark contrast between Florida’s hot weather and Chicago’s snow for the first time. I graduated from the University of South Florida with a major in Biomedical Sciences. Throughout my medical education I’ve been privileged to contribute to restoring health and well-being. This path has not only shaped my understanding of medicine but also deepened my appreciation for the interconnectedness of science, compassion, and human dignity.  Outside of work, I love traveling, staying active with workouts and sports, and dining at rooftop restaurants with beautiful views. Having lived in underserved communities, I’ve encountered their challenges firsthand, which inspires me to give back to the community and advocate for healthcare equity. 

Michael Fuller, MD

Hello everyone! My name is Michael. I was born and raised in Little Rock Arkansas. I took a non traditional path to becoming a physician, as I graduated with a bachelors in Business and worked in medical sales for some time which led to realizing my calling in Family Practice.  I completed medical school at St Georges University where I received my medical degree, and completed my clerkship training between Bronxcare Hospital and Flushing Hospital in New York City.  I am very passionate about the aspects of longevity and preventative care within family medicine that can be leveraged to help patients proactively combat chronic disease.  In my free time I enjoy playing basketball, boxing, shooting pool, cooking, and golf.  I’m excited to train here at Humboldt Park Health and look forward to this next adventure. 

Muhammad Asad Khan, MD

Hello everyone! My name is Asad and I am from Karachi, Pakistan. After completing my medical education from Karachi, I worked on multiple public health education initiatives before moving to North America with my wife. I enjoy going to the gym, swimming, listening to old music, and cooking new foods. I am excited and grateful for the opportunity to train at Humboldt Park and to participate in their mission to advance health equity

Shivani Sareen, MD

Greetings! My name is Shivani Sareen, and I am from Toronto, Ontario. I am a proud alumni of Saint James School of Medicine, where I completed my medical training. Having spent considerable time in Chicago for my clinical rotations, I have grown fond of this city. Post-graduation, I served as a USMLE Counselor in Chicago, guiding and mentoring students towards their professional aspirations. With a strong passion for education, I find immense joy in both teaching and learning. I am excited to join Humboldt Park Health for my residency, driven by the opportunity to provide care to underserved communities. Beyond medicine, I indulge in live music, dancing, and Netflix. As an avid coffee connoisseur, I am always up for discussing the perfect brew!

Rody Chehade, MD

Hi! I’m Rody. I was born and raised in Lebanon, and I completed my medical degree from St. George’s University. Chicago has become my second home now, and I’m looking forward to living here for the next few years! Fun facts about me is that I am a Karate black belt, played semi-pro soccer in Chicago, and traveled to more than 15+ countries around the world.

Hello! I’m Sakshi Bhardwaj, originally from Alberta Canada. This is my first time in Chicago, and I’m excited to explore this vibrant city known for its rich history, diverse neighborhoods, and incredible food scene. I’m passionate about community-centered care while also being committed to working with underserved populations to help improve access to quality healthcare. In my spare time, I enjoy Pilates, reading, and hiking. I’m excited to grow both personally and professionally during my time here and to contribute meaningfully to the Chicago community.

Giovanni Carrillo-Becerril, MD

I completed my medical degree at Tecnológico de Monterrey in Mexico, where I developed a strong interest in both clinical care and research. I have participated in projects ranging from cancer immunology to cardiology, but I found my greatest fulfillment in community-focused care. I am particularly passionate about working with underserved populations and exploring approaches to aging and proactive medical care. When I was looking for a residency, what stood out to me the most about this program was the people—the sense of support, teamwork, and community made it feel like the right place to grow as a physician. Outside of medicine, I enjoy reading, watching movies, and playing the piano.

Muhammad Usman Khalid, MD

Hi, I’m Usman! I was born and raised in a small town Haroonabad, in Punjab, Pakistan and am honored to be the first physician in my family. My journey through medicine has been shaped by experiences in both urban hospitals and underserved rural clinics, which inspired my passion for Family Medicine and community-focused care. I chose Chicago-and Humboldt Park Health-because of its vibrant diversity, welcoming spirit, and commitment to serving communities that remind me of home. Outside of the hospital, I enjoy stand-up comedians (wanna-be-one), cooking (therapeutic and spicy), and creating memes-yes, I actually do it professionally. Try me! I also have a deep love for storytelling, whether through humor or heartfelt conversations, and I believe a good laugh is often part of good healing. I’m excited to continue growing as a physician in a place that values empathy, innovation, and human connection just as much as I do.

Ahmad Kiani, MD

Hi! My name is Ahmad Kiani, and I am from Mississauga, Ontario in Canada. I completed undergrad at McMaster University where I received my Bachelor of Science degree and later attended medical school at St. George’s University where I received my MD. During medical school, I rotated at HPH where I had an incredible experience taking care of a diverse patient population with an amazing team. I’m excited and grateful for the opportunity to train at HPH to provide care for underserved communities. When I’m free, I enjoy going on long drives and exploring new places. I am also a Formula 1 fan and always up to try a new escape room. 

Vladislava Manoilova, MD

Hi! My name is Vladi, and I’m originally from Edmonton, Alberta. I completed my medical education at St. George’s University and spent my clinical rotations in Chicago. I chose family medicine because I’m passionate about building long-term relationships with patients and making a meaningful impact in underserved communities. In my free time, I enjoy drawing and painting, working out, and spending time with my two cats. I fell in love with the city during my time at Humboldt Park Health, and I couldn’t be more grateful to continue my journey here as a resident in a community that already feels like home.

My name is Joseph Moss. I was born and raised in Chicago, and attended medical school at Ross University school of medicine. I’m excited to be back home and serve this community. I chose family medicine because of the great need of practitioners who are able to treat diverse conditions and prevent chronic illness. In my free time I enjoy weight lifting and reading.

Marta Sciubisz

Program Director

I graduated from American University of the Caribbean School of Medicine and I completed my residency in Family Medicine. Prior to joining this residency program, I was working in group practice. I’m passionate about preventive care and engaging patients in their own care and decision making. Outside of work I enjoy numerous outdoor activities including skiing, biking and hiking and many others. And when the weather is “too exciting” for my outdoor adventures, I’m happy to read a good book or go out to a movie theater.

Samir Shahen, MD

Associate Program Director

I am born and raised in Chicago, therefore was excited to complete my family medicine residency at St. Mary and Elizabeth Medical Center, in Chicago, serving as a Chief Resident in my last year of residency. I am board certified in Family Medicine and have been working as a member of the Hospitalist Program and a faculty member for the Family Medicine Residency teaching service. I’m an active member of the American Academy of Family Medicine (AAFP), American Board of Family Medicine (ABFM), and in the Society of Teachers of Family Medicine (STFM). I have a strong interest in working in an underserved community as well at teaching I enjoy watching Chicago sport teams, working on cars, traveling and playing recreational soccer.

Jia Meng, MD

Hello! I grew up in the Chicago suburbs. I started out earning my degree in Finance from the Geis School of Business before realizing that healthcare was really where my heart was. After attending Case Western Reserve University School of Medicine, I completed my Family Medicine residency in Lima, Ohio, where something unexpected happened – I found out I really enjoyed teaching my fellow residents and medical students! That discovery, combined with my curiosity, made me realize that medical education was exactly where I belonged.

Outside of work, I’m a bit of a techie who loves building computers. You’ll also catch me obsessing over the perfect espresso shot, playing sports, and gaming with friends. These hobbies create a sense of satisfaction outside of work and help me connect with residents and patients who share similar interests!

Lisa Tran, MD

Faculty

I was born and raised in Milwaukee and began my healthcare career as an ICU nurse after earning my BSN at Milwaukee School of Engineering. I completed medical school at St. George’s University, where I did most of my clinical rotations at Humboldt Park Health, and went on to Family Medicine residency at Mercy Health – St. Rita’s Medical Center in Lima, Ohio, serving as Academic Chief Resident in my final year. I am board certified in both Family Medicine and Obesity Medicine. My clinical interests include obesity medicine, pediatrics, prenatal care, preventive health, and the management of complex chronic conditions. I chose to return to Humboldt Park Health to teach and practice because of my commitment to serving the community that helped train me as a medical student. Outside of medicine, I enjoy traveling with my husband, cooking, exploring Chicago, and walking our corgi, Boomer.

Sammir Naser, MD

Hi, I’m Sam. I was born and raised in Cincinnati, Ohio.  I was a “non-traditional” medical student because I had a career before this venture. In 2008, I graduated with a bachelor’s of business administration before moving to NYC for 5 years. After a series of events, I realized my career path was ready for a shift, at which point I decided to pursue medicine. I completed the MD/MPH dual degree program at SGU and my clerkship at Humboldt Park Health. Now, I’m excited to begin my career as a family medicine attending physician in the same program!

Nadia Kalam

I’m Nadia! I grew up on the North Side of Chicago and spent most of my adult life in the city before recently relocating to the suburbs. Throughout my career, I’ve had the privilege of working closely with residents in various capacities—and it’s work I truly enjoy. One of the most rewarding aspects of being part of Graduate Medical Education is the opportunity to meet and collaborate with people from all walks of life. Every year brings a new group of individuals, and watching their growth over the course of three years is something I take great pride in. It’s incredibly fulfilling to support and witness that transformation. I’m also proud to be part of the close-knit, family-like environment we’ve cultivated within our team. Outside of work, I enjoy spending quality time with my family and friends, playing with my pups, cheering on the Cubs, and discovering new restaurants around this amazing city.

My name is Leslie born and raised in Chicago. I been in the healthcare field for a couple years and I also had the opportunity to work at our HPH clinic before becoming the Medical Education Coordinator. I enjoy working with the students and interacting with their excitements as they learn in different specialties. I am a buffalo wild wing lover while watching UFC, Football and baseball and Watching a horror movie every weekend is a must for me!
